Overview of a health record:
Each health record is made up forms, folders, and attachments.  Descriptions of each of these components are given below:

Forms :
Lab test forms: There are over 50 different lab test forms available. You select the type of test you have done and the corresponding blank form opens. For example, if you had a urine test done and want to upload those details, select urine tests from the dropdown list.

Surgery details form: This form has basic details of the surgery such as:

    • Date of surgery
    • Where the surgery was performed,
    • The surgeon’s name and contact address
    • A short description of the surgery.

     

     

    Prescription details form: This form has details of prescription such as

    • The disease diagnosed,
    • Drugs prescribed
    • The schedules for taking that medication.

    Hospitalization details form: This form has details of hospitalization such as

    • Admission and discharge date
    • Hospital name, address

    Doctor’s visit details form: Has details of doctor’s visit such as

    • The date of visit, 
    • Doctor’s name and address,
    • The disease suspected
    • The final diagnosis

    Health check up details form: This form has details of the health check-up such as

    • All the lab test reports
    • All the prescriptions
    • Ahe suspected diagnosis
    • The final diagnosis

    Folders: Folders contain forms.
    The different types of folders available are

    Doctor’s visit folder: If you have visited your doctor and want to upload the details/records of that visit, click on the “Doctor’s visit” folder. This folder contains sub folders for storing details of the drugs prescribed and Lab tests prescribed by the doctor.

    Hospitalization folder: If you were hospitalized and you want to upload the details/records of that hospitalization, click on the “Hospitalization” folder. This folder contains sub folders for surgeries done, drugs prescribed, and Lab tests prescribed during the hospitalization

    Health Check-up folder: If you had a health check-up done and you want to upload the details/records of that health check-up, click on the “health check-up” folder. This folder  contains sub folders for drugs prescribed, and lab tests done

    Minor Surgery folder: If you had a minor surgery such as “surgery for an ingrown toenail, or a ear piercing” and you want to upload the details/records of that minor surgery, click on the “minor surgery” folder. This folder, contains sub folders for drugs prescribed, and Lab tests prescribed during that minor surgery

    Only Prescriptions folder: If you only want to store all the doctor’s prescriptions you ever had in a separate folder (without details about the doctor’s visit), then click on the “Only prescriptions” folder. This folder allows you to store only prescriptions in a separate folder (only for those prescriptions).

    Only Lab Tests folder: There may be times when you are asked to get just a couple of diagnostic test done, for instance, many companies these days ask their employees to do a few such diagnostic tests as “medical tests.” If you only want to store all the lab/diagnostic in a separate folder (without details about the doctor’s visit/ hospitalization/health check-up), then click on the “Only lab tests” folder. This folder allows you to store only diagnostic/lab tests in a separate folder.

    Attachments:
    Besides filling up the fields in each form, you can also scan your medical report and upload it as an attachment. (jpg,jpeg, gif file, 300 dpi resolution, maximum size 400 kb per file) to that form. For example, besides entering the details of your blood test report into the online form, you can scan your blood test report and upload it as an attachment. Similarly, you can scan and upload all your X-rays, MRI Scans, ECG report… etc.

    Which type of health records can I upload?
    You can upload any kind of health record such as a prescription of drugs, surgery report, X ray, MRI Scan, ECG Report and all reports of diagnostic tests done such as blood test, urine tests, stool tests… etc.

    How much space do I get?
    You get upto 10 MB space

    When can I view my records?
    After you have opened an account and uploaded your records, you can use the “View Records” link to view the records you uploaded. You can also use the “search” link to find and view the particular record you are seeking.

    Give your doctor access. You can also give your doctor access to your records. This means that even when you are traveling abroad, you (and your doctor who is abroad), can view the health records that you uploaded.

    How can I give my doctor access to view my records?
    Send your username and password to your doctor via e-mail address or cell phone, along with the website address http://www.healthizen.com .

    If I’m traveling abroad can I, or my doctor in that foreign country, view my health records?
    Yes. Ask your doctor to go to http://www.healthizen.com and log in with your username and password.

    How do I upload my records?

    The steps to upload your medical records are:

    1. Sign in using your username and password
    2. Click on the “My Health Records” Link in the left navigation bar
    3. Click on the “Upload Records” link
    4. Select from among the folders there which are: “Doctor’s visit”, “Hospitalization”, “Health Check-up”, “Minor Surgeries”, “Only prescriptions”, and “Only Lab tests.”
    5. Fill the forms in each folder with the relevant details that are in your printed health records. For example, use the printed copy of your doctor’s prescription to fill in the same details into the online “prescription form.”
    6. Click the “Save” button to save all details to the server.

     

    P.S.: You can also scan your prescription or any other printed health record and upload the scanned file as an attachment in a .pdf format, jpg or gif. (JPG file should be in max 300 dpi, and 50 kb)

    I have an X-ray OR an MRI Scan. How do I upload it?

    Reports such as X rays, MRI Scans, ECG graphs, and other such graphical reports can be scanned and uploaded as an attachment.

    I visited the doctor yesterday and he gave me a prescription of drugs, how do I enter the doctor’s prescription?

    1. Sign in using your username and password
    2. Click on the “My Health Records” link in the left navigation bar
    3. Next, click on the “Upload Records” link in the center panel
    4. Now click on the “Only prescriptions” link

    Fill the form you see there.
    When you have finished, click “save folder”

    Alternatively, you can click on the “Doctor’s visit” l and enter the    prescription details along with the details of your doctor.

    1. In the “Prescription Details Form”, enter the following:

      • Drug name: This is the name of the drug prescribed, for e.g.: Your doctor may have prescribed a drug called “Zagam” so enter “Zagam”
      • Generic drug name: While “Zagam” is the current brand name of the drug and when you go to the pharmacy you will ask for “Zagam.”  However, the generic name for “Zagam” is “Sparfloxacin.” To find the generic name, look for it on the packaging in which the drug comes.
      • Administration Method: From the dropdown list select whether the drugs are to be administered Orally, Intra-Venous, or in some other manner.
      • Form: From the dropdown list select whether the drugs are to be consumed in tablets, syrup… or in some other form.
      • Dosage quantity : Enter the dosage quality. For example, 250 mg/ 5 ml…etc(mg, g, ml)
      • Frequency: Enter the frequency
      • Duration: Enter the duration
      • Instructions: Select one of the options from the dropdown list
      • Comments: Enter any comments such as…”watch for contraindications/allergic reactions…” etc. You may also use this field to type in details which are in your prescription but could not be fitted in to any of the fields mentioned above
      • Attachments: You can also scan the prescription and upload the scanned image or pdf file as an attachment. (image files should be jpg, jpeg, or gif files, in 300 dpi resolution, maximum size per attachment = 150 kb)

    My doctor asked me to do a blood test. I have the blood test report. How do I upload the details of diagnostic tests such as blood tests, X rays, MRI scans…etc?

    1. Sign in using your username and password
    2. Click on the “My Health Records” link in the left navigation bar
    3. Next, Click on the “Medical Records” link
    4. Now, click on the “Upload Records” link in the center panel
    5. Now click on the “ Only Lab tests” link
    6. In the “test type field” select “Haemogram” from the dropdown list of tests. ( A blood test is also called a Heamogram)  

    1. Fill the blood test form and all the fields (such a W.B.C. count, R.B.C count, hemoglobin, etc) that are displayed. After you have filled in all the fields you can also upload a scanned image of the blood test report, as an attachment. When you have finished, click the “Save” link
    1. Alternatively, if this blood test is part of a doctor’s visit, you can click on the “Doctor’s Visit” folder and enter it as part of the lab tests section there, along with other details of your doctor.
    2. Attachments: You can also scan the blood test, X ray, MRI scan…etc and upload the file as an attachment. (image files should be in 300 dpi resolution, maximum size per attachment = 150 kb. File formats allowed are .jpg, .jpeg, .gif, and .pdf files)

     

    I have had a small but painful wart removed from my foot, how do I upload the details of a minor surgical procedure like this?

    • Sign in using your username and password
    • Click on the “My Health Records” link in the left navigation bar
    • Next, Click on the “Medical Records” link
    • Now, click on the “Upload Records” link in the center panel
    • Now click on the “ Minor surgeries” link
    • Fill the details in the form there. You can use the remarks/comments field to fill in any details that do not seem to fit into the other fields.

    I was hospitalized last month. How do I enter the details of my hospitalization?

    • Sign in using your username and password
    • Click on the “My Health Records” link in the left navigation bar
    • Next, Click on the “ Medical Records” link
    • Now, click on the “Upload Records” link in the center panel
    • Now click on the “ Hospitalization” link

    Fill all the forms you see there.
    Note: This folder allows you to save your prescriptions, of surgery (if any), and the lab test results; as part of this hospitalization folder, so that you don’t have to save them separately.
    Attachments: You can also scan all the hospitalization records and upload the scanned image or .pdf file as attachments. (Image files should be .jpg, .jpeg, or .gif files, in 300 dpi resolution, maximum size per attachment = 150 kb).

    How do I view a record that I uploaded?

    To view the records you just uploaded, Click the “View records” link

    How do I edit a record that I just uploaded?

    To view the records you just uploaded, Click the “View records” link, locate the record/s you want and click the “edit” link (look for the pencil icon) to edit each record. When you have finished, upload the edited record to the server.

    How do I get a print copy of the record I just uploaded?
     
    To get a print-out of any record you uploaded, click on the “View records” link. Here you view a list of all records. You can then click on each individual record to view it and click on the printer icon to get a print out of each such record.
